Nurse's Office

At TMSA Triangle, the health and well-being of our students are our top priority. We are committed to fostering a safe, supportive, and nurturing environment where every student can learn, grow, and thrive. Our school community works together to promote healthy habits, prevent illness, and ensure that students receive the care and attention they need throughout the school day. By partnering closely with families and healthcare providers, we aim to protect our students’ overall wellness and create a school environment where every child feels safe, valued, and ready to succeed.

Health Assessment and Vaccination Requirements

There are several health documents that all North Carolina public schools, including charter schools, are required to collect and report annually.

If you are NEW to TMSA, you must submit the following forms during registration:

Every parent is still required to submit the necessary immunization documentation directly to the Nurse’s Office annually. Please email all records to Nurse Oates at coates@tmsacademy.org.

Garrett's Law

Garrett’s law was enacted in 2004. It mandates that NC schools provide parents and guardians with information about meningococcal meningitis and influenza, as well as the vaccines that protect against these diseases. The law was expanded in 2007 to mandate that information also be provided about human papillomavirus (HPV) and the vaccines available to protect against HPV.

Meningococcal Meningitis
Influenza (Flu)
Human Papillomavirus (HPV)

Vaccination ExemptionThere is required documentation that must be submitted if your student is exempt from the state’s vaccination requirements.

  • If a student is unable to receive a required immunization for medical reasons, families must complete the NC Medical Exemption Form, which must be signed by a licensed healthcare provider.
  • If a family declines immunizations for religious reasons, they must complete only the TMSA Religious Exemption to Immunization Form and submit it to the Nurse’s Office.

Health Decision Guide for School Attendance

Medication Policy

For the safety of all students, the Nurse’s Office follows strict guidelines regarding medication.

  • We do NOT administer any medication without proper medical documentation from a licensed healthcare provider.

  • A parent or guardian note is not sufficient.

  • This policy applies to all medications, including common over-the-counter items such as Tylenol, Motrin, cough drops, antihistamines, allergy tablets, ointments, and any homeopathic remedies.

To request that medication be administered at school, the required medical form must be completed and signed by your child’s healthcare provider. You can access the form HERE.

Keep Us Informed

Communication is key to ensuring your child receives the best care and support while at school. The more information we have about your child’s health needs, medical conditions, or recent illnesses, the better prepared our team will be to respond appropriately and collaborate with you. Please keep us informed at all times — your updates help us maintain a safe and healthy environment for all students.

  • Please notify Ms. Teja (tmudunuri@tmsacademy.org) of any changes in home, cell, or work phone numbers.

  • Please notify Ms. Oates (coates@tmsacademy.org) of any changes in your student's health needs.

  • We kindly remind families that school staff cannot provide extended sick care. Students who become ill must be picked up promptly. Refusing or delaying pickup is not permitted, as it compromises the student’s well-being and the health of others.

  • Please report your child absence on Infinite Campus, and if necessary, submit a doctor's note upon the students return to school.
